Need Transportation?

If you need transportation for yourself, a family member, patient, or resident, please use our online Ride Request Form.

Provide your pickup location, destination, requested transportation date, appointment time, and return-trip information, and our team will contact you to review availability, trip details, and pricing.

Submitting a ride request does not confirm a reservation. Transportation is confirmed only after you receive confirmation from Florida Lakes Medical Transport.
